If you have a problem or question about this online form or process, CALL MY CELLPHONE at 630-479-0001 ONLINE School Application Form All groups should apply beginning at 9 a.m. The first group of high school jazz ensembles to be accepted will be SECOND or THIRD ensembles from schools with more than one jazz ensemble. THOSE GROUPS HAVE PRIORITY FOR SELECTION. (Only one jazz ensemble per school will be accepted.)
Performance times not taken by these SECOND or THIRD highs school ensembles by 1 p.m. today will be reserved for INEXPERIENCED FIRST AND ONLY ENSEMBLES accepted in the order of the transmission time this application was submitted beginning at 9 a.m.. If your school has more than one ensemble, your FIRST ensemble may not participate. (Any junior high school group can participate.)
APPLICATIONS THAT ARE INCORRECTLY COMPLETED WILL BE THE RESPONSIBILITY OF THE DIRECTOR. EVERY YEAR WE HAVE A FEW SCHOOLS THAT ARE DISQUALIFIED BECAUSE THEIR APPLICATIONS SAID THAT THE APPLYING BAND WAS "1st and Only", BUT THE DIRECTOR SAID ON THE FORM THAT THERE WERE MORE THAN ONE JAZZ ENSEMBLE IN THE MUSIC PROGRAM. These applications were discarded since it is impractical for us to contact everyone for clarification.
SCHOOLS MAY ONLY BRING ONE BAND TO PERFORM IN OUR FESTIVAL. (You may bring additional bands or students to observe for FREE, but those students will be charged full price for their evening concert tickets if they stay.)

JAZZ DIRECTORS BIG BAND: NOTE CHANGE FOR THIS YEAR ONLY We will again present our popular "Jazz Directors Big FAT Band" to perform on stage from 2:30 p.m. to 2:50 p.m. (No rehearsal- music will be sent in advance).
This year only: The "Jazz Directors Big FAT Band" will be used as a demonstration group for a clinic presented by James Warrick at the upcoming Midwest International Band Clinic in Chicago at 2:30 p.m. on Thursday afternoon, December 18, in the Grand Ballroom. ALL MEMBERS OF THE "DIRECTORS FAT BAND" WHO PERFORM AT THE MIDWEST CLINIC WILL RECEIVE A FREE PARTICIPANT BADGE TO THIS YEAR'S MIDWEST CLINIC in advance of clinic week. Preference for membership in the "Jazz Directors Big FAT Band" will be given to directors who can also perform at the MIDWEST CLINIC on December 18. Do you wish to perform in the "Jazz Directors Big FAT Band" AT THE MANTOOTH FESTIVAL on FEBRUARY 7, 2009? YES NO
Will you be able to perform in the "Jazz Director's Big Fat Band" at the MIDWEST INTERNATIONAL BAND CLINIC on Thursday afternoon, December 18 in Chicago? (You will get complimentary "Participant" registration for the entire Midwest Clinic.) YES NO

WE NEED THE FOLLOWING INFORMATION NOW SINCE TICKETS GO ON SALE SOON. WE NEED TO KNOW IF YOU ARE STAYING FOR THE EVENING CONCERT, AND THE MAXIMUM NUMBER OF TICKETS YOU WILL NEED TO AVOID OVERSELLING THE AUDITORIUM. (Last year all 1,535 tickets were sold by late November.) You can adjust your total number DOWN later, give us the maximum number of tickets you will need NOW. (You cannot ask for more tickets later, only fewer than you list below.)
We must limit the number of REDUCED PRICE EVENING CONCERT TICKETS ($20) we can sell to our guest schools to the number of students actually performing, and chaperone(s). We will still provide one complimentary director ticket. We cannot accommodate evening concert requests for reduced price tickets to bring additional non-performing bands or additional non-performing students from one school. Those non-performing students are welcome to attend the free daytime clinics and concerts, but if they wish to attend the evening concert they must pay for a full price ($30) a ticket. Those tickets must be ordered using this online application. WILL YOUR GROUP STAY FOR THE EVENING CONCERT? YES NO If so, how many REDUCED PRICE tickets @ $20 each to see THE COUNT BASIE ORCHESTRA will you be ordering for your PERFORMING students and chaperones to attend the evening concert? (Note: The actual ticket order and money will not be required until NOVEMBER 19, 2008. Slight changes in the number of tickets DOWNWARD you order now are possible at that time. Tickets for the general public are $30 each.) MAXIMUM quantity of REDUCED PRICE tickets @ $20 each for your PERFORMING STUDENTS/chaperones your school will need. (Include a director's complimentary ticket in this total.) This number can be reduced later if necessary. IT CANNOT BE INCREASED LATER.

Do not mail your participation fee with this form. You will be billed according to your performance location.
A request for a full refund of the participation fee must be received in writing by schools that withdraw prior to January 5, 2009. Schools withdrawing after that date will forfeit the entire fee. FEES TO BE BILLED (and paid by November 3, 2008):
Since all stage spaces are taken by schools who pre-registered donating used instruments to NOLA, there is only one performance fee this year...
Check your information carefully. Every year there are several bands who are disqualified because the submitted application said that the applying band and was "1st and Only", but later in the form the director said there was more than one jazz ensemble in the school. RETURN TO THE TOP and hit the "SUBMIT" BUTTON ONE TIME.
