Donation procedure for
LOCKS OF LOVE
We will make a pair of reserved seats on the main floor available for no
charge to anyone who donates a qualifying amount of hair to LOCKS OF LOVE
between the dates of August 1, 2007 and January 15, 2008.
Procedure
to receive tickets
1. Review
the requirements for donations on the Locks of Love website.
2. Download
the donation form (PDF) from Locks of Love.
3.
Follow their donation procedure and send you hair donation directly to Locks
of Love.
4.
On the form there is a "Comments" section. In that section write
"Donated on behalf of the Frank Mantooth Jazz Festival at New Trier
High School."
5.
Mail or fax a photocopy of the donation form to...

Once we receive your form we will contact you about the tickets to see if
you wish to purchase additional tickets.

The donor's name will be printed on the Jazz Festival's webpage and in the
festival's program.
