Please make sure you have submitted the following forms on time. They are crucial to our planning process.

DUE Friday, January 13th
"Breakfast, Lunch, and Setup" form is due. This form allows us to order the right amount of food, and helps us plan equipment usage for all our visiting schools.
"T-Shirt order form" (optional) - The Jazz Fest TShirt looks really cool this year (design is below). If your students would like to order TShirts, please submit this form by Friday, January 13th so I can order enough shirts.
